Registration amounts:
$350 per attendee
$250 Active Duty Military Attendees
$250 for Pensacola Bay Area Chamber Member.
$275 for Central North Florida HIMSS Chapter Member.

Payment in US Funds, drawn on a US bank, must accompany this for your registration to be processed. For additional registrations please make a copy of this form (each registration must be filled out separately)

Persons under the age of 18 are not permitted at the Strategic Health Intelligence Summit.
